Ch. 1, § 4. Fee for Revised License

If, after a health care facility is issued a license, it changes its name, location, or the number of beds, the health care facility must pay a fee to cover the cost of issuing revised license. (a) Revised License fees shall be assessed at $50 per health care facility.
