I, Deponent's name, W/o Sh. Husband's name, D/o Sh. Deponent's father's name, aged about Deponent's age years, resident of Deponent's address, do hereby solemnly affirm and state on oath as follows:-

1.That my maiden name is Maiden name.

2.That I was married to Sh. Husband's name on Date of marriage at Place of marriage.

3.That after my marriage, my name is Married name, and all records bear the same name.

4.That my Aadhaar number is Aadhaar number and my PAN number is PAN number, which have remained the same before and after my marriage.

5.That I have not obtained a marriage certificate.

6.That I state that Maiden name and Married name are the names of one and the same person, namely myself.

7.That I am executing this declaration for submission to the concerned authorities for the purpose of change of my name in their databases / records.

8.That I hereby state that whatever is stated above is true to the best of my knowledge and belief.
