AFFIDAVIT
I, Deponent's first name Deponent's surname, son of Deponent's father's first name Deponent's father's surname, aged about Deponent's age years, resident of House No. House number/House sub-number, Ground Floor, Block number, Locality, Sub-locality, PIN code, do hereby solemnly affirm and state on oath as follows:-
1.That I am the father and natural guardian of my daughter, namely Daughter's name, who is my first-born child, and her place of birth is Daughter's place of birth.
2.That the true and correct date of birth of my daughter, Daughter's name, is Daughter's date of birth. I am applying for her admission to Class Class applied for in Name of the school for the Academic Session Academic session.
3.That the deponent has no documentary proof regarding the date of birth of his daughter, namely Daughter's name.
4.That due to inadvertence, I did not register the date of birth of my daughter, Daughter's name, with the concerned department at the time of her birth.
5.Hence, this sworn affidavit is being made in lieu of a date of birth certificate.
