R6-5-5003. Notification of Changes

A. A provider listed on the CCR&R database shall notify the Contractor of any changes to the information or statement given under R6-5-5002(A) or (C)(1). B. A provider may modify self-initiated changes in listing status at any time by notifying the Contractor.
