R6-6-1806. Contents of a Request for Administrative Review

A. A request for Administrative Review shall include: 1. Identification of the Action; 2. Reason for the request for administrative review, including why the Requestor disagrees with the Action; 3. Desired resolution; and 4. Written consent of the Applicant, Member, or Responsible Person, when applicable. B. The Division shall consider additional supporting documentation submitted by the Requestor or Representative within 10 days of the file date of the request for an Administrative Review. The Division may consider additional supporting documentation submitted by the Requestor or Representative more than 10 days from the file date of the request for an Administrative Review.
