603 DENIAL OF APPLICATION FOR LICENSE ─ NOTICE 603.1 If an application for a license or a renewal of a license is denied, the Department shall provide the applicant with written notice that includes: (a) The specific reasons and legal authority for denial of the license; (b) The actions, if any, that the applicant must take to qualify for a new license or to renew a license; and (c) Notice of the applicant’s or licensee’s right to a hearing as prescribed in Subsection 812.3.
