5095 – Tracking and Trending of Data (Rev. 28, Issued: 10-21-16, Effective: 10-21-16, Implementation: 10-21-16) QIOs must conduct analyses of data from all review types to identify potential Quality of Care Concerns in accordance with the contract requirements. Data analyses may be conducted within and/or across provider settings in accordance with contract requirements. In addition, QIOs must make every attempt to identify Quality of Care Concerns related to health disparities issues, including racial, ethnic, or socioeconomic indicators. NOTE: When a potential Quality of Care Concern is identified through data analysis, a QIO should conduct a General Quality of Care Review. (See 42 CFR §476.160(a)(1)(iii) and §§5100- 5115.3 of Manual Chapter 5.)
