HAR §17-1722-121. Eligibility reviews

(a)
Eligibility reviews shall be conducted at intervals
determined necessary by the department.

(b) Individuals who fail to complete or fail to
cooperate in the completion of the eligibility review
shall be ineligible for benefits under this program.

§17-1722-122 State pharmacy assistance program
card. Program members may be issued a state pharmacy
assistance program card from the department or the
