(e) A facility operator shall submit a payment request for each receipt of funds in the form prescribed by the corporation. As applicable, the payment request shall include a computation of the funds sought, and an attestation that the services for which the payment is being requested are being and will be satisfactorily rendered and that all contractual obligations are and will be duly fulfilled.
