HAR §16-76-7. Application form for license

An application for a license to practice
chiropractic shall be submitted on a form or forms prescribed and provided by the
board, accompanied by the evidence, statements, documents, and all applicable fees as
are required, and filed with the board at its office in Honolulu. [Eff 3/16/73; am and ren
