Consent To Release Confidential Information

Participants entering a substance abuse treatment program must sign a consent to release program information to th e Department. The treatment program will only release substance abuse treatment information to report participan t progress. (3-17-22) 167. FAILURE TO COMPLY WITH SUBSTANCE ABUSE SCREENING AND TESTING REQUIREMENTS. TAFI applicants or participants refusing to cooperate with substance abuse screening, assessment, testing or treatmen t are ineligible. (3-17-22)
