Section 739.165Â Tracking
a)        Acceptance. A used oil burner must keep a record of each used
oil shipment accepted for burning. These records may take the form of a log,
invoice, manifest, bill of lading, or other shipping documents. Records for
each shipment must include the following information:
1)Â Â Â Â Â Â Â Â The name and address of the transporter that delivered the
used oil to the burner;
2)Â Â Â Â Â Â Â Â The name and address of the generator or processor from whom
the used oil was sent to the burner;
3)Â Â Â Â Â Â Â Â The USEPA identification number and Illinois special waste
identification number of the transporter that delivered the used oil to the
burner;
4)Â Â Â Â Â Â Â Â The USEPA identification number and Illinois special waste
identification number (if applicable) of the generator or processor from whom
the used oil was sent to the burner;
5)Â Â Â Â Â Â Â Â The quantity of used oil accepted;
6)Â Â Â Â Â Â Â Â The date of acceptance; and
7)Â Â Â Â Â Â Â Â If the transporter has accepted any
shipment of mixtures of used oil and materials identified in 35 Ill. Adm. Code
808.121(b)(5) or (b)(6), the following:
A)Â Â Â Â Â Â Â Information
stating when and where the special waste was generated;
B)Â Â Â Â Â Â Â The classification and quantity of the
special waste delivered to the transporter;
C)Â Â Â Â Â Â Â Any special handling instructions
pertinent to emergency personnel in the event of an accident; and
D)Â Â Â Â Â Â Â A generator's certification as
follows:Â "I hereby declare that the contents of this consignment are
fully and accurately described above by the proper shipping name, and are
classified, packaged, marked and labeled/placarded, and are in all respects in
proper condition for transport according to applicable international and
national governmental regulations. If export shipment and I am the Primary
Exporter, I certify that the contents of this consignment conform to the terms
of the attached EPA Acknowledgement of Consent. I certify that the waste
minimization statement identified in 40 CFR 262.27(a) (if I am a large quantity
generator) or (b) (if I am a small quantity generator) is true."
b)        Record Retention. The records described in subsection (a)
must be maintained for at least three years.
