Section 117.220Â Service
facilitation services
a)Â Â Â Â Â Â Â Â The Department shall notify individuals who are chosen to
participate in the program in writing of the availability of an array of
community services which Department-designated agencies can provide, including
service facilitation.
b)Â Â Â Â Â Â Â Â Periodically, as desired by the individual but no less than
annually, the service facilitator shall review with the individual the adequacy
of the plan and make any modifications desired by the individual.
