Service/treatment plan

Section 117 Section 117.225  Service/treatment plan a)         The service/treatment plan shall be developed by the individual, his or her guardian, and other persons the individual asks to participate as well as a service facilitator. b)         The plan shall serve as the basis on which services may be billed by an agency, organization or individual  and reimbursed by the Department.
