LAC 43:V.968. Decommissioning Report [Formerly LAC 43:I.1168]

A. Within 60 days after lessee has completed the decommissioning requirement and has restored the lease site by the removal of all alternative energy source equipment, including, but not limited to, facilities, turbines, support structures, cables, or pipelines, lessee shall submit a written report to the board, through OMR, that includes the following: 1. a summary of the removal activities, including the date removal activities were completed; 2. a description of any mitigation measures taken by lessee; 3. if lessee used explosives, a statement signed by lessee’s authorized representative certifying that the types and amounts of explosives utilized were consistent with those in the approved decommissioning application.
