LAC 52:I.1308. Disclosure Forms Filed Pursuant to R.S. 39:1233.1

A. Disclosure forms filed pursuant to R.S. 39:1233.1 shall: 1. be in writing and on the form in §1910; 2. contain: a. the name and address of the public servant; b. the public position held by the public servant; c. the name and address of the bank; d. the position held with the bank by the public servant and whether that position is compensated or noncompensated; and e. a description of the transaction from which the public servant recused himself from participating; and 3. be signed by the public servant. B. The executive secretary shall maintain these forms suitably indexed.
