Voluntary Requests for Department Services

(1) Any person located within the Commonwealth may request services from the Department. 110 CMR 4.01 through 4.09 apply to requests for Department services. In order to request Department services, the person making the request must complete a written application form. The Department shall provide assistance to any person who needs assistance completing the written application form.
