PAUL PLACE, BALTIMORE, MARYLAND 21202-2272 WRITER’S DIRECT DIAL: 410-468-2205 Facsimile Number: 410-468-2204 e-mail: hmax@mdinsurance.state.md.us BULLETIN To: Insurers, Nonprofit Health Service Plans, and Health Maintenance Organizations with Provider Panels Re: Credentialing of Providers by a Credentialing Intermediary Date: November 20, 2002 Bulletin No.: Life and Health Bulletin #02-25 Senate Bill 819, Chapter 153 of the Acts of 2002, states that the Insurance Commissioner may permit a carrier to use a health care facility’s credentialing form to credential providers at the health care facility if the carrier has designated the health care facility to be the credentialing intermediary for the health care facility’s physicians. A separate filing with the Commissioner will not be required for a carrier to use a health care facility’s credentialing form. Any carrier may use a health care facility’s credentialing form provided: 1) it is used only to credential providers at the facility; and 2) the facility has been designated the credentialing intermediary by the carrier. Carriers are required to use the Uniform Credentialing Form to credential all other providers. Questions about this Bulletin may be directed to Kimberly Y.
