PCIDDT (Provisional Certification) Application
A. To apply for temporary certification as a PCIDDT, an individual should submit
an online application which contains the following:
1. PCIDDT Application Form;
2. PCIDDT Verification of Employment Form;
3. Official Transcript; and,
4. Application Fee - $90.00 fee
B. Application forms and application components (i.e., educational escripts and fees) are
submitted online as prescribed by the Mississippi Department of Mental Health via the
agency (DMH) website.
