CIDDT (Full Certification) Application A. To apply for full certification as a CIDDT, an individual should submit an online application (initial or upgrade) which, at a minimum, contains the following: 1. CIDDT Application Form; 2. CIDDT Verification of Employment Form; 3. CIDDT Verification of Work Experience Form; and, 4. DMH Intellectual and Developmental Disabilities Therapist Examination Documentation: The Division of PLACE maintains examination score reports; therefore, there is no reporting requirement for successful completion of the DMH Intellectual and Developmental Disabilities Therapist Examination. As appropriate, however, an applicant should designate in the appropriate space on the application form that he/she has successfully completed the examination requirement. B. If the CIDDT applicant is submitting an initial (not upgrade) application, the following additional CIDDT application component(s) are also required: 1. Official Transcript – (If upgrading from PCIDDT, no additional transcript is required.); and, 2. Application Fee - $90.00 fee (If upgrading from PCIDDT, no application fee is required.) Only individuals applying directly for CIDDT pay the application fee. This is a ONE TIME fee; applicants should not pay the application fee twice. C. Application forms and application components (i.e., educational escripts and fees, if applicable) are submitted online as prescribed by the Mississippi Department of Mental Health via the agency (DMH) website.
