Any complaint that a violation has occurred shall be made as follows:

Any complaint that a violation has occurred shall be made as follows:
(1)
By submitting within sixty (60) days of when the Complainant knew or
should reasonably have known of an alleged violation, a completed online
complaint form adopted by the Board, which shall include at a minimum:
(a)
the name, address, telephone number, and email address of the
complainant
(b)
the date, location, and nature of the alleged violation
(c)
a description of all facts and evidence supporting the complaint
(d)
documentary and photographic evidence supporting the complaint
(e)
any other information that the complaining party considers relevant
(2)
The alleged violation form shall also include, to the extent known by the
Complainant, the name, address, telephone number, and email address of
the Respondent
(3)
By submitting a complaint in any other method adopted by the Board.
