Address/location (An Inpatient Hospice facility must notify and receive approval by Mississippi State Department of Health prior to a change of address/location); 2. Agency name; 3. Phone number; 4. Hours of operation/24 hour contact procedure; 5. Change in address or phone number of any branch office; 6. Administrator; 7. Director of nursing; and 8. Cessation of business.
