- 15 MAC Pt. 16, R. 1.7.2Provisional License – Within its discretion, the Mississippi State Department of
- 15 MAC Pt. 16, R. 1.7.3A hospice program against which a revocation or suspension proceeding is
- 15 MAC Pt. 16, R. 1.8.1A Hospice shall not be operated in Mississippi without a valid license from
- 15 MAC Pt. 16, R. 1.8.2Any person or organization desiring to operate a hospice shall file with the
- 15 MAC Pt. 16, R. 1.8.3The application shall include complete information concerning the address of the
- 15 MAC Pt. 16, R. 1.8.4Ownership of the hospice shall be fully disclosed in the application
- 15 MAC Pt. 16, R. 1.8.5A license shall be issued to the person(s) named only for the premises listed on
- 15 MAC Pt. 16, R. 1.8.6Licenses are not transferable or assignable
- 15 MAC Pt. 16, R. 1.8.7Each planned change of ownership or lease shall be reported to the Department at
- 15 MAC Pt. 16, R. 1.8.8The application is considered a continuing application
- 15 MAC Pt. 16, R. 1.8.9Fees: Prior to review for an initial license and prior to license renewal, the facility
- 15 MAC Pt. 16, R. 1.8.10Operational Requirements/Conditions of Operation – In order for a hospice

- 15 MAC Pt. 16, R. 1.9.1Mississippi State Department of Health shall be notified, in writing, of any of the

- 15 MAC Pt. 16, R. 1.14.1The hospice shall maintain operational policies and procedures, which shall be
- 15 MAC Pt. 16, R. 1.14.2Such policies and procedures shall accurately reflect a description of the hospice’s
- 15 MAC Pt. 16, R. 1.15.1Personnel Policies – Each licensed hospice agency shall adopt and enforce
- 15 MAC Pt. 16, R. 1.15.2Personnel Records – Each licensed hospice shall maintain complete personnel
- 15 MAC Pt. 16, R. 1.15.3Criminal History Record Checks
- 15 MAC Pt. 16, R. 1.15.4Staffing Schedule – Each hospice and alternate site shall maintain on site current
- 15 MAC Pt. 16, R. 1.16.1Contract Services – Contract services may be provided when necessary to
- 15 MAC Pt. 16, R. 1.16.2The hospice must assure that contracted staff are providing care that is consistent

- 15 MAC Pt. 16, R. 1.18.3NOTE: The orientation and training curricula for hospice aides shall be detailed
- 15 MAC Pt. 16, R. 1.18.4Licensed Practical Nurse (LPN) - The LPN must work under the direct
- 15 MAC Pt. 16, R. 1.18.5Medical Director/Physician Designee - A physician, currently and legally
- 15 MAC Pt. 16, R. 1.18.6NOTE: The Medical Director or Physician Designee may be an employee or a
- 15 MAC Pt. 16, R. 1.18.7Occupational Therapist
- 15 MAC Pt. 16, R. 1.18.8Occupational Therapy Assistant (OTA) Qualifications – The occupational
- 15 MAC Pt. 16, R. 1.18.9Physical Therapist (PT) - The physical therapist when provided must be

- 15 MAC Pt. 16, R. 1.25.7Staffing standards for inpatient hospice care provided through a contract may not
