Renewal/reinstatement of Licensure
1.
The license holder shall request a renewal/reinstatement application from the
Department in writing within five (5) years of the year of retirement.
2.
The license holder shall submit the renewal/reinstatement application, all lapsed
license renewal fees, and the continuing education requirement to the Department
for review.
