Complaints: all complaints concerning a certified individual, his business, or professional practice, shall be reviewed by the department. Each complaint received shall be logged, recording at a minimum the following information: 1. Name of individual certified; 2. Name of the complaining party, if known; 3. Date of complaint; 4. Brief statement of complaint; and 5. Disposition.
