For a new certificate to be prepared and issued, the following shall be furnished to the State Registrar immediately after adoption: 1. Certified copy of final decree of adoption. 2. Report of Adoption, a form prescribed and furnished by the State Registrar. The report shall include such facts as are necessary to locate and identify the certificate of birth of the person adopted, provide information necessary to establish a new certificate of birth of the person adopted, identify the order of adoption and be certified by the clerk of the court. The Report of Adoption must be complete and meet the requirements of an acceptable record under
