Disclosure of non-identifying information from the sealed file shall be made only under the following conditions: 1. The person making the request has sufficient proof of identity and is an adoptee eighteen (18) years of age or older; an adoptive parent; the guardian or legal custodian of an adoptee; or the offspring or blood sibling of an adoptee if the requester is eighteen (18) years of age or older. 2. Payment of the required service fee.
