Department Forms. Applications, requests, notices, reports, certifications, and other submissions required under this Manual shall be submitted on forms prescribed by the Department. The Department may revise forms, instructions, filing formats, and submission requirements as necessary. Department forms and instructions shall not supersede applicable statutes, this Manual, or the State Health Plan.
