COMMONWEALTH OF PUERTO RICO # OFFICE OF THE COMMISSIONER OF INSURANCE CIRCULAR LETTER AV-III-8-935-83 October 4, 1983 TO ALL INSURERS AUTHORIZED TO TRANSACT LIFE AND DISABILITY INSURANCE IN PUERTO RICO Re: Insert Pages which amend or substitute pages of previously approved policy forms Dear Sirs: Reference is made to our Circular Letter AV-III-10-897-82 of October 22, 1982, regarding the above captioned subject. This Office will accept the submission for approval of insert pages which intend to substitute or modify other pages of a previously approved policy form conditioned to the following: 1. Each insert page must be identified in the lower left or right hand corner, with the form number of the policy to which it is made part of. 2. The date of revision must also appear in the lower left or right hand corner of each insert page. Notwithstanding the above, it must be clearly understood that any change to a previously issued policy must be made by means of endorsements or riders pursuant to Section 11.250 of the Insurance Code of Puerto Rico, and the same will be subject to the approval of this Office.
