R436-2-3. Foundling Certificates

(1) The Office of Vital Records and Statistics (office) shall include on the foundling certificate form provided by the office: (a) information from the hospital that receives an infant of unknown parentage found in the state, including: (i) the date and address of the place of finding, which will be used as the place of birth; (ii) the sex, approximate live birth date, and race of the infant if known; (iii) the name and address of the person or institution submitting the report; and (iv) other medical information, if available; and (b) the name for the infant obtained from the Division of Child and Family Services. (2) Information submitted under this section shall constitute the basis for the report of live birth for the infant.
