Ch. 4, § 3. Provider Invoice

(a) The Division shall provide a written invoice to the provider and parent for signature. The invoice shall contain at least the following information: (i) Provider name, address, and phone number; (ii) Rate of payment; (iii) Rate of parent co-payment; and (iv) Duration.
