AFFIDAVIT

I, Deponent's name, S/o Deponent's father's name, aged about Deponent's age years, resident of Deponent's address, do hereby solemnly affirm and state on oath as follows:-

1.That I am the father and natural guardian of my daughter, namely Daughter's name. She is my first-born child, and her place of birth is Daughter's place of birth.

2.That the true and correct date of birth of my daughter, Daughter's name, is Daughter's date of birth. I am applying for her admission to Class Class applied for in the School for the Academic Session Academic session.

3.That the deponent has no documentary proof regarding the date of birth of his daughter, namely Daughter's name.

4.That due to inadvertence, I did not register the date of birth of my daughter, Daughter's name, with the concerned department at the time of her enrolment.

5.Hence, this sworn affidavit is being furnished in lieu of a date of birth certificate.
