704 ISSUANCE OF POOL AND SPA OPERATORS’ REGISTRATION CARDS 704.1 To qualify for a Pool and Spa Operator’s Registration Card an applicant shall: (a) Complete a registration form provided by the Department of Health; (b) Submit proof of certifications specified in Section 300; and (c) Pay the required fee. 704.2 A Pool and Spa Operator’s Registration Card shall be valid for a three (3) year period, as specified in Subsection 301.4.
