Pike, Insurance Commissioner Date: September 5, 2024 Subject: Filing a Complaint Against Another Title Licensee The Insurance Department requires documentation and evidence to investigate allegations of misconduct when they arise. Complaints against other licensees are no different. Effective immediately, a title licensee’s complaint against another title licensee shall be submitted to the Insurance Department by filing a completed Title Licensee Complaint Form. The form can be found on the Department’s website at https://insurance.utah.gov/formsreports/#forms. The form must be completed in its entirety and must include documentation to support the complaint’s allegations. The Department cannot guarantee the confidentiality of any information submitted.
